For patients without dental insurance, Morgan Family Dental offers an In-Office Dental Plan for individuals and families. This plan can help patients save a significant amount of money on dental care by eliminating the high costs and hassles associated with billing insurance companies. We offer a Standard Plan, bene ting anyone with standard dental care, and a Comprehensive Periodontal Plan
for individuals with a diagnosis of gum disease.

Terms and Limitations
Enrollment fees paid for each person must be paid in full on or
before their first visit and are non-refundable. Enrollment is per person and active for 365 days from the date of enrollment. Consecutive years will not be auto-renewed and will require additional enrollment paperwork. The plan enrollment fee is valid for the current year and is subject to change thereafter.
All treatment must be paid in full at each visit to qualify for 20% or
25% discount. Treatment not paid in full will be charged at full
fees. Any benefit not used during an enrollment period cannot be
carried over.
In-office Dental Plans are not transferable to others and are not valid towards any treatment due to an injury involving a lawsuit, car accident, medical insurance, workman’s compensation, or homeowner’s insurance.
This is an In-Office Dental Plan and is NOT dental insurance. It
cannot be combined with any other discount plan or dental
insurance. Membership is only good at the office of Morgan Family Dental: 16670 Franklin Trail S.E. Prior Lake, MN 55372. Treatment outside our office is not included in the In-Office Dental Plan.
